You will receive a replacement pair of clear lenses from the same lens supplier and in the same prescription, design, index, coating and frame*.

How long do patients have to exchange?
Patients have 30 days from collection of eyewear to exchange for a clear pair from the same lens manufacturer and in the same prescription, index, design, coating and frame.
Simply write the date of collection on the Satisfied or Exchanged card that will arrive with the lenses from your lab. Give the card to the patient with their new Transitions lenses.

What do I have to do when patients exchange?
If patients return to your practice, take back their Transitions lenses. Fill out the Claim & Return Form and make a copy of their proof of purchase/collection. Return the Transitions lenses with the Claim & Return Form and the copy of proof of purchase/collection to your laboratory.
If you do not return the Transitions lenses immediately, simply order replacement clear lenses from your lab in the same prescription, design, index and coating as the original Transitions lenses. The cost of the clear lenses will be credited when the returned Transitions lenses, Claim & Return Form and proof of purchase/collection are received by your laboratory.
When returning Transitions lenses always submit the Claim & Return Form and proof of purchase/collection all at the same time.

What will it cost me?
There is no cost to the practice. Your lens manufacturer will charge you for the Transitions lenses when you initially order them.
When exchanging Transitions lenses, return the Transitions lenses with the Claim & Return Form and a copy of the patient’s proof of purchase/collection.
If you do not return the Transitions lenses immediately, simply order replacement clear lenses from your lab in the same prescription, design, index and coating as the original Transitions lenses. You will be charged for clear lenses. You will be credited the clear lenses once your laboratory receives the returned Transitions lenses accompanied by the Claim & Return Form and the proof of purchase/collection. Transitions lenses returned to your laboratory must ALWAYS be accompanied by the completed Claim & Return Form and the proof of purchase/collection. There is no refund of the price difference to the patient.
Certificate of Authenticity card
A Certificate of Authenticity card will be inserted with every pair of Transitions lenses from your lab. Simply write in the collection date and hand to your patient along with their new and improved Transitions lenses.
