This guarantee also includes Transitions® performance sunwear; if you are not satisfied with their performance you can simply exchange them for a fixed tint lens from the same supplier, in the same prescription and frame*.

How long do patients have to exchange?
Patients have 30 days from collection of eyewear to exchange for a clear pair when returning Transitions adaptive lenses or static sun lenses when returning Transitions performance sunwear from the same lens manufacturer and in the same prescription, index, design, coating and frame.
Simply write the date of collection on the Certificate of Authenticity card that will arrive with the lenses from your lab. Give the card to the patient with their new Transitions lenses and receipt of purchase.

Certificate of Authenticity card
A Certificate of Authenticity card will be inserted with every pair of authentic Transitions lenses from your lab. Simply write in the collection date and hand to your patient along with their new Transitions lenses and receipt of purchase.
What do I have to do when patients exchange?
Fill out the Claim & Return Form and make a copy of their proof of purchase/collection. Return the Transitions lenses with the Claim & Return Form with a copy of proof of purchase/collection to your laboratory.
If you do not return the Transitions lenses immediately, you can order replacement clear or static sun lenses from your lab in the same prescription, design, index and coating as the original order. However, the cost of the clear or static sun lenses will only be credited to you when the returned Transitions lenses, Claim & Return Form and proof of purchase/collection are received by your laboratory.

What will it cost me?
There is no cost to your practice. Your lens manufacturer will charge you for the Transitions lenses when you initially order them.
To exchange the Transitions lenses, return the Transitions lenses with the Claim & Return Form and a copy of the patient’s proof of purchase/collection.
If you do not return the Transitions lenses immediately, you will be charged for the replacement order of clear or static sun lenses from your lab in the same prescription, design, index and coating as the original Transitions lenses order.
You will be credited the lenses once your laboratory receives the original Transitions lenses accompanied by the Claim & Return Form and the proof of purchase/collection. Transitions lenses returned to your laboratory must ALWAYS be accompanied by the completed Claim & Return Form and the proof of purchase/collection. There is no refund of the price difference to the patient.
